About us:
Streamline Psychiatry, LLC was found with the purpose to bridge the gap in a continuum of mental healthcare setting over a time continuum as patients and providers adapting to evolving landscape in healthcare. Under founder of Hoa Le, D.O., we strive to provide excellent care to patients across a continuum of healthcare setting in rural as well as in urban community over the patient lifespan using innovative and collaborative approach. Specifically, through use of in-person and telehealth services, patients receive easy access to mental health services in different healthcare setting over a time continuum while maintaining independence within the community and an enhanced quality of life.
Our Mission:
Streamline Psychiatry, LLC devotes to growing a network of exceptional mental health providers as resources to providing excellent care to our patients across a continuum of care setting in rural and urban community. Our providers dedicated to evaluate, support, and provide treatment to adult and growing aging population with a spectrum of mental health condition that include depression, anxiety, obsessive-compulsive disorder, PTSD, bipolar, schizophrenia, ADHD, substance use disorder, and among other. By utilizing biopsychosocial model, our providers are expected to derived a personalized treatment plan to meet our patients’ goal and objectives. Moreover, as caregivers are a vital part of our care team, our providers are dedicated to working collaborative with caregivers as our patients retain control and stability of the highest quality of life.
Hoa Le, D.O.
Hoa Le, D.O. is the founder of Streamline Psychiatry. LLC. He is a practicing adult and geriatric psychiatrist with experiences in treating patients across the continuum of healthcare setting over the patient lifespan in the rural and urban community. His experience further extended to caring for active-duty military members and veterans. He is Board Certified in General Psychiatry and Geriatric Psychiatry by the American Board of Psychiatry & Neurology. His research interest includes the progression of cognitive and functional ability in schizophrenia over a time continuum. His other area of study and interest includes psychodynamic psychotherapy.
