Our network of providers are devoted to providing medical and mental health services to our growing aging population who suffer with Major Neurocognitive Disorder, also known as dementia. Our services extend  across a continuum of mental health care settings that include outpatient clinic, inpatient hospitalization, day program, primary care clinic, emergency room, inpatient medicine consult, nursing home, home care, and other. As our network of providers grows, our patients and caregivers can expect the support and highest quality of care across a continuum of care setting over our patient lifespan.
 
Our services:
- Conduct initial comprehensive psychiatric assessment utilizing biopsychosocial model
- Perform screening for neuropsychiatric symptoms
- Perform screen for cognitive and functional decline
- Perform medical and neurological examination
- Provide accurate and concise diagnosis, treatment plan, disposition, and referral
- Provide ongoing treatment and follow-up visit
- Provide social services for patients, family members, and caregivers
- Provide telehealth services
- Provide home visits
- Provide referral for home health service
- Provide opportunity for patients and caregivers to participate in clinical research
